Who is UNI Coordinate?

We are a veteran-owned care coordination partner committed to helping healthcare providers and organizations extend care beyond the clinical visit.

We are nurses, physicians, certified case managers, health educators, and administrators. We understand the challenges providers face when patients need additional support navigating referrals, appointments, community resources, care transitions, and treatment plans.We work alongside healthcare providers and organizations to strengthen the connection between the provider’s recommendations and the patient’s next steps. Through structured follow-up, practical problem-solving, clear communication, and personalized support, we help patients overcome barriers while keeping healthcare teams informed.Our goal is not simply to complete individual coordination tasks. We help organizations develop dependable care coordination processes that support continuity, improve follow-through, and create a better experience for patients, providers, and the communities they serve.
